Description :
The West Island Health and Social Services Center (WIHSSC) is currently looking for a Nurse Practitioner to fill a full-time rotating day/evening vacancy with the Aire ouverte team at CLSC LaSalle.
The mandate of Aire ouverte is to co-develop a network of integrated youth services by building partnerships with youths, families and caregivers, facilities and stakeholders from within the CIUSSS, as well as community organizations and other institutions. These services will address the mental, physical and sexual health needs (holistic approach) of vulnerable individuals between the ages of 12 and 25 and of their families/caregivers in a community setting that is welcoming and non-stigmatizing. Team members will therefore have an opportunity to:
- Participate in broadening the service offering with the aim of providing intervention methods that are varied, flexible, and adapted to young people (e.g., appointments, walk-ins, community outreach, telephone and virtual consultations with both young people and partners, IT);
- Offer brief one-on-one or group interventions;
- Create and follow a professional training plan to be well equipped for this role;
- Work as part of a multidisciplinary team (e.g., nurses, psychologist, sex therapist, psychoeducator, occupational therapist, art or music therapist, peer helper, etc.) and with community and institutional partners.
Specialized Nurse Practitioners (SNP) are nurses who, upon completion of their graduate studies, have acquired advanced knowledge in nursing and who, through their training, are authorized to perform certain medical activities. They use their specialized and advanced knowledge and expertise to make complex decisions, based on a comprehensive and holistic approach, to address the needs of patients and their families in their specific area of expertise. By virtue of their academic background, they perform advanced practice functions that allow them to improve and promote quality health care more efficiently.
Main duties and responsibilities:
The SNP who will join the Aire ouverte team will work with adolescents and young adults as well as with their families/caregivers, alongside a multidisciplinary team of professionals. Through by-appointment or walk-in consultations, he/she will:
- Participate in broadening the service offering at Aire ouverte
- Perform comprehensive health assessments of patients upon their arrival
- Order and interpret diagnostic tests
- Prescribe medical treatments and medication
- Promote health through healthy lifestyle practices
- Work in partnership with the patient and their family
- Plan and coordinate care
- Ensure continuity of care
- Serve as a resource person and support the care team
- Liaise with the various internal and external partners
- Ensure client follow-ups
- Provide professional leadership: interdisciplinary training, training of future IPSSM (nurse practitioners specialized in mental health)
- Consult with physicians involved with Aire ouverte on complex cases as needed.
Requirements :
The ideal candidate will have:
- A Bachelor’s degree in Nursing, at least 2 years of practical experience in his/her specialty practice area, a graduate level degree allowing one to work as a SNP, as well as the SNP certificate (see the AIPSQs website for more information: https://bit.ly/2U5uvaN);
- Experience working with adolescents and young adults is an asset;
- A driver’s licence and access to a vehicle to get around for work purposes;
- The ability to get around in order to work in various locations across the territory covered by the Montréal West Island CIUSSS;
- The ability to communicate in English and French, both orally and in writing;
- The skills to use a computer and computer programs;
- The ability to work during Aire ouverte’s business hours, which include some afternoons, evenings and weekends.
